Constant night wake ups at 16 months

Please can you help me work out how to break some negative sleep patterns?
My son was hospitalised for two weeks in December with pneumonia. He is 16 months. He was very ill. Throughout the time in hospital he and I shared a bed. Before he got sick he had his own cot.
Since coming out of hospital he has been waking every hour through the night. He needs both milk in a bottle and to resettle on me to get back to sleep. He’s now well again. I don’t do anything other than cuddle him and provide milk (fresh bottle each time) to get him back to sleep but it’s dusrupting my life (I work full time) and it’s not sustainable.
If I put him in his cot awake or in a light sleep he cries and throws himself around, quite often banging his head.
I need to get him to sleep in his own cot and I need to get him to resettle himself.
Can anyone help me devise a gentle plan to do this whilst I’m exhausted and need sleep so I can work.
