How do we get DS in his crib?

DS is 13 weeks (8 weeks corrected) bottle fed although not from birth. He has severe silent reflux and mild cmpi. He is neocate and rantidine gaviscon. He has never really slept well unless upright. The only place he would sleep for a short period early evening till about 2/3am was his pram carry cot. He will not settle at am in his crib on his back. I have had a sling which has been invaluable during the day, out and about as I have a 4 year old too. He has been sleeping on me and my husband more and more. However he is now not settling in his carry cot either now and will only settle when picked up. I go to bed at 9-10 and up at 3 when I swap with my husband. We are currently shifting in the lounge. We have two bedroom bungalow and rooms quite close together as DS has been disturbing dd. I start my day at 3/4 as he will not settle on his own after this. He won't settle in our bed as won't be flat on his back. However his naps are falling to pot too as he won't settle on his own. Occasionally he will nod off in his bouncy chair but will only nap for 20 mins at a time , after becoming over tired. He wakes himself up and if we put him down he wakes himself up too! Obviously we need to try and get him sorted however his reflux is not sorted although slightly better and his was premature in scbu for 17 days. What do we do? I tired and DH is we haven't slept in same bed for weeks. However we can't let him scream it induces horrendous reflux episode ( we have done this) however he can't sleep on us. My DH has stag dos and work conferences coming up so I need to he some sleep. His crib is raised and makes no difference. Dd had reflux it slept with us and was in her crib settling for part of the night by now. Any ideas advice welcome! Does his prematurity feature? He doesn't like tummy and his head control isn't great - I think due to prematurity. We have a plan with his reflux consultant led. But have the feeling this is as good as it will get.

He now won't settle in his carry cot early in the evening now whereas he was before. He will only settle on one if us. He's been wide awake at 3-4ish tio past two nights. But he was asleep in sling a lot of today as we went out for most of the day. We can't afford to keep losing sleep my DH drives for a living and I went into the back if someone in the car but it was very minor I think my attention span is short!
