Settling 16 month old who has never slept through

Please help! I'm exhausted and don't know what else to try!

My 16 month old DD2 wakes up at least once, but usually 2-3 times a night. I usually feed her (though have weaned during daytime), and she sometimes falls asleep straight away, but more often takes a while, sometimes a couple of hours, to settle. We (usually me!) put her back in her cot, pat her or rock her to sleep. She can't / won't fall asleep on her own. Sometimes she wakes up, has a little cry and then goes back to sleep on her own, but this is quite rare. I've tried night weaning her but she will just scream and wriggle until I give in and feed her.

With DD1 we used pick-up-put-down, which worked really well, but I think DD2 is too old for it. It just seems to make her more agitated.

Is there anything similar to PUPD for older babies that we could try? I want to avoid letting her cry it out, tried with DD1 but just couldn't go through with it.
