10 month old bad sleeper

DS is now 10 months and is not a great sleeper. He rarely sleeps for more then 3 hours. If I breast feed him he will go straight back off but if I don't then it's an hour or more of wriggling, crying, fussing etc.
he just wriggles around in my bed, sucking his thumb and fussing then when he is almost almost cry nearly asleep - he pulls his thumb out and cries! Goes on for at least an hour. Then he will sleep for an hour maybe 2 and repeat the whole process.
Breast feed him and he will sleep for 2-3 hours and then repeat.
But I'm shattered from the endless wakings.

I want to teach him to sleep and to re settle.
He does go to sleep by himself but by laying on the bed with me. Then I transfer him to his cot.

I don't know how to teach him to sleep and resettle!
