15 month poor sleeper, need to stop co sleeping help!

Hey everyone. Desperate for a good nights sleep here :(

DS is nearly 16 months old & has always been crap sleeper. At around 5 months out of sheer desperation we co slept and are now stuck in the habit. However now he's older we are getting kicked all night & it's just not comfortable.

I haven't tried any method of sleep training consistently as he's teethed pretty much one after the other. - just cut 4th canine so only 4 teeth left to go, he's also suffered with constipation so we are now on long term medication & he seems ok.

So normal night goes like this:
630pm bath pj's relax in bedroom
7pm milk & cuddle, put him down & sit by cot with hand on him till he nods off, takes anything from 5-30 mins
9-11pm resettle
11pm-12 usually wants milk (I know he shouldn't need it)
3-4am ends up in bed with us because I'm knackered.
8am up for day

He has one pm nap of around 1-1.5 hours.

I'm a bit of a wuss with controlled crying but I'm at the point where I'm willing to try anything to get a better nights sleep.
