6 month old - trouble settling

My dd is 6 months old and she always waked at bedtime after 30 mins of sleep. She will then settle if I go into her room and put her dummy in and hold her hand, then after another 30 mins its the same all over again. This continues until about 10pm when she then sleeps through until about half 6/7am.

Is there something I can do to stop these initial 30 minute wakings?

Her naps throughout the day are 1 30 min nap about 9am, about and hour and half around 11/12pm, and then another 30-1 hour nap around 3/4pm. I start the bedtime routine at 5.45 with a bath, then pjs, then bottle (formula) then put her into cot tired, and she will only settle with dummy and me holding her hand.

I would tey to gradually stop the hand holding and dummy. For the hand holding maybe first of all just try to not be holding when she actually falls asleep and then over a number of nights try moving further from the cot. For the dummy I think there are some good ideas in Elizabeth Pantley's "No cry sleep solution" book.
