Is something actually wrong or am I just not being tough enough?

DD is 18 months and a shite sleeper. At the moment she goes to bed at 7ish no problems 90% of the time, then wakes around midnight droning and crying, continues this for a couple of hours minimum, then sleeps on and off until 6 or 7am. She's howling with tiredness by 11am and naps 2-3 hours some days.

When she wakes at night she seems restless, twitchy and throws herself around the cot. DS was the same but developed sleep apnoea and eventually had his tonsils and adenoids out, which helped enormously. DD is starting to snore but no apnoea so far.

Does this pattern sound 'normal', ie is it fair to sleep train her if her sleep seems disturbed rather than she's simply waking for a quick feed (which she gets anyway but it doesn't help with the crying!)

Sorry, how long and dull Blush

Not dull at all. Take her to the gp and explain and see if they can check her over for you or refer to ENT. If you have any doubt that it is discomfort that is keeping her up then you shouldn't do sleep training as you prob won't see it through. Could adenoids, apnoea run in families?
