19 month old sleep nightmare!

DS2 is 19 months and for the past 3 nights he has been up wailing angrily for hours and hours.
I don't think he's teething, he doesn't seem to be in pain and he's not hungry. He's still breastfed but we nightweaned a while ago and he has generally slept through for about 6 months now.
He still has 2 naps during the day which we try to limit to about an hour (he would sleep more as he has a heart condition which means he gets tired quite quickly).
DH and I are knackered. Is it just a case of this too shall pass?

I would switch to 1 nap after lunch at this age at around 12.30pm. Its possible with the 2 naps he isn't tired enough at BT to sleep a full night through.
