No sleep!!!! Help!

My DD is 41 weeks old. She is really active during the day, crawling quickly, standing up unaided - but not quite walking yet.

She was breastfed up until 8 and a half months but now has a bottle before each sleep (AM, PM and Last feed)but no feeds during the night.

BUT her sleeping is a real problem. She has 2 naps during the day, one in the morning and one in the afternoon - on EVERY occasion she will only fall asleep if she is being cuddled. The minute I put her down she wakes and screams until I pick her up again.
Night time is a similar story - she falls asleep after a bottle in either mine or my husbands arms and then when we try to put her in her cot she suddenly wakes up and screams until we pick her up again. Eventually we can get her into the cot whilst she's asleep if we're very quiet but she will often wake about 1am an then we can't get her to go back to sleep in the cot, however if we put her in bed with us she goes straight to sleep and will sleep through the rest of the night.

This is what we've tried so far:
Putting her in the cot before she falls asleep
Establishing a routine - bath etc (still in place)
Warming the blankets in the cot before putting her down
Letting her cry it out a little bit - but she has real staying power - SCREAMS non stop

Nothing is working. It is really affecting our sleep and of course our love life.
I return to work in 3 weeks and I'm really worried that I'm going to be too exhausted to do the job.

I know it's not the place to say this, but I had the same until DD was 6 months & I hired Alison Scott Wright who sorted it all out very quickly. If you arne't in a position (or dont' want to) hire someone to help (she stayed 3 nights with us, although 2 would've done it) it might be worth getting her book from the library. Don't know the title, but is on Amazon etc.
