I bought one for my sister and 2 dd last xmas but fom a Calendar shop (the sort thats open only at christmas time) so they do exhist! If you're near Doncaster (long shot!) it was at the lakeside outlet. I'm looking for another so will keep watching this thread and let you know.

We just fill the five columns - two for my DD as she seems to have far too big a social life, so more room to write, and a final column labelled "birthdays"!

Yes, there's a company who print individually to order with as many columns as you want and names of your choice at the top of eavh column. I've seen them linked to on here before; will see if I can find the old post.

I saw a 3 column one in (I think) the Lakeland catalogue yesterday - unfortunately it was a couple one with two wide columns and a thinner one headed Birthdays. It also had little pockets on each month for invites etc. I was tempted, despite the couple bit.
Just have a hunt around a bit nearer to xmas, I am sure there will be loads - at the end of the day a 4 column one, with one of the columns for birthdays would work well.

Have found the link: Kitchen Calendars. You choose the number of columns, a name for each column, the start month and the number of months.
