Cloth nappying from birth

We're expecting our first in the summer and planning to use cloth from birth. At the moment we've got a mixture of newborn sized nappies and btp (totsbots and little and bloomz). Any advice on using cloth from birth? Specifically thinking...

Under/over the cord? Is this easy with a cloth nappy?
Liners? Fleece or disposable? (If disposable, how do you dispose of them)
Set up - were really keen to make this work so advice on optimum set up/routines would be really helpful!

No liners needed until weaning (as long as you’re not using nappy cream of using a cloth friendly cream) as you can throw it all in the machine until then! We still only use (fleece) at night as we use nappy cream at bedtime and they help keep his bum a bit drier.
