SIF Forms for C of E Secondary

So we have a number of forms for a few churches which cover our worship attendance for the last 15 years. The forms have varying frequencies and durations.

Does anyone have any experience of working out entry points and able to offer any advice?

I’m not sure what you are asking. Surely you know how often you attend worship on average, e.g. once pw vs once a month, once a term etc. If they are looking back 15 years, I presume it is a sixth form admission, in which case perhaps details of confirmation etc.

The number of "points" you get for attendance will vary depending upon the exact admissions criteria for the schools you are applying for, so you need to look closely at each of those and figure out into which criteria you fit for each school. (I assume that is what you are talking about!)

You need to check the criteria for the schools concerned. When we filled out forms for Y7 entry, the maximum time they were interested in was the past 10 years. they weren't interested in attendance before dc was born. We had attended 2 churches in that time and so the forms had to be filled in by the ministers/vicars at both.
The minister / vicar ticked the boxes for which category we were in for admissions for both churches.
