My family and I are Anglican, but my DD wants to become catholic. I support her wholeheartedly, but I do not know how to apply for one? We have been Anglican our entire lives

You will need to find out who is the priest in your parish and contact them.
It not an easy process. It involves attending lessons and and having ceremonies.
My child attended a Catholic school as an Anglican but wanted to convert. The process took nearly a year.

Yes, you need to attend buts it's fine if you don't covert. I am baptised Anglican but am not religious. The church were aware of this.
My son attended classes with either the priest or the nun at the church and I stayed with him. They reminded me of Sunday school classes to be honest 😂
He did that every Sunday for a few months and then attended a ceremony which I can only describe as a bit like a baptism where he got a certificate. After that he had to wait till the next group of children were having their communion and he attended with them.
