Inclusion settling in support?

Does anyone have experience of this at secondary transition please? My DD has never had any concerns at primary and now I've received a message from her new secondary saying a detailed questionnaire from her primary school suggests she may need 'inclusion settling in support'. I feel really worried what it could mean since there have been no issues before.

Might it be it is a questionnaire sent to all parents, to ascertain which pupils need 'settling in support ?

Or

Might it be that they have got her mixed up with another child or a similar name ?

I'd give them a ring and ask - say that Primary school have never mentioned any concerns about her socialising so you just wanted to check if this is for everyone or if they've got the wrong person.

Your child may have expressed a worry about making new friends, losing old ones and any stories circulating about the new school.

Are her primary friends going to different schools? It might be a case of them wanting to make sure those who come up without friends settle in and find friends. Is she a child who might be daunted by the size of the place, or organising herself for lots of different teachers?
I would guess that she's on a list of "keep an eye out to check these ones settle okay".
