I got called a fat overweight c* by my fiancé about 3 weeks before we got married. He’s since apologised and said he was drunk and didn’t mean it. I’ve got a mum tum but I’m not overweight, he wonders now why I have very low self esteem and confidence…..

It's not his words though that gave you low self esteem, you must have this already that you entered into a relationship with him to the extent that you are engaged to marry such a horrible loser.
His comments highlight your low self confidence but did not cause them. A person with a healthy self esteem just wouldn't have given this guy the time of the day let alone be in a relation with him.
You are with losers like him and stay after hurtful comments because you must think he is the best you could get, you don't deserve better and this is your fault. Don't marry the bastard.

That was a really shitty thing for him to say to you OP. Was it in the context of a huge row, not that that makes it acceptable, but would add some sort of context when he'd say something like that. The main thing is, after he apologised what has he done to repair that damage? Anything? If not, then I'm not sure what to tell you except make it clear to him how hurtful that was and see his reaction, and if he actually seems genuinely sorry. If he calls you names regularly or starts that shite up again I would seriously reconsider staying married to him though.
