Jane Slalom Pro tipping over

I had a very scary experience yesterday.My 19 month woke up quickly and must have moved up the pram to the top.The pram tipped backwards and she slipped right out on the wooden floor,getting a very nasty bump.Surely prams shouldn't tip back like that?
Has anyone else had this problem?

Most prams would do that. 3 wheelers especially.
Did she have harness on?
My DS1 tipped DS2 backwards in buggy and he was fine, if a little shocked.

that shouldn't happen if she is harnessed in properly and securely. Were there any bags hanging from the handle as weight will destabilise three wheelers very easily as they are already back-heavy.

She had a harness on yes,but she slipped through it.There wasn't anything attached either,her weight when she crawled up to the top must have tipped it.
I won't be letting her sleep in it again!
