Advice needed: patio doors with a flush threshold for wheelchair user

I have also posted this in the elderly parents forum, but may get more traffic here.

We need to replace the patio doors (from dining room to the conservatory) for doors with a flush threshold, so my mum (a wheelchair user) can access the conservatory by herself. They are currently sliding doors, but have a threshold of 12.5cm, and we have had a quote for patio doors with a 2.5cm threshold, but we have mocked it up and my mum can’t even wheel over that, even with little ramps either side. Anybody got any advice for types of doors / companies (in the midlands)? Thank you.

We are also looking to replace existing upvc French doors from the kitchen to the garden, and will need a completely flat threshold for that as well. Thanks in advance :)

There’s actually an article on exactly this in this month’s (June) Ideal Home that describes the problems (difficulty with rubbish getting in the groove if sunk below floor level).
