Concealed bathroom fittings and repairs

I'm currently in the early stages of planning out my bathroom refurb and I really like the look of wall mounted fittings, but I'm worried about how you get to them if they need repairing. In my current 15+ years old bathroom, the taps are mounted on the bath and the sink, so they're easy to get to if there's a leak. But how does this work with wall mounted taps and toilets? Is there an access panel? Can they be repaired without having to remove/damage tiles?

There are 2 main types of wall mounted taps.
One type the tap is connected to a copper pipe coming straight from the wall.
The whole tape can be replaced from the wall.

The other will have a concealed mixer behind the wall.
The thing most likely to fail is the cartridge; that can be replaced from the front of the tap.
