Anyone know what could cause this to happen

I believe there are quite a few possible causes.
In my case I think it was the fact I had 7 healthy 8-9lb babies and by the time I got number 8 to 29 weeks my womb just had enough!!

My first was following a car crash when I was 26 weeks. The second at 24 weeks due to complications with growing my baby and a placenta in my damaged uterus (damaged following the birth of ds1) there wasn't enough room basically in between all the metal holding my body together. Ds2 s birth led to a emergency hysterectomy.
It is rare, I was told trauma or physical/genetic issues with the womb or placenta are the leading causes.

Mostly it is unknown. My daughters first baby had this at 37+4 weeks. The only indication things were slightly off was the 36 week scan noted he was small for dates where previously had been fine. DD is not tall so they weren't worried. DGS was 4.5 lbs after emergency c section.

Quite often they just don't know.
