Mistake on my health and pregnancy grant form...Advice!

I saw the GP yesterday for a checkup (who scared me shitless into sending me off for a growth scan today, but that's another story!). Anyway, I forgot to ask for the Health in Pregnancy Grant form when I was there. I rang the surgery once I'd remembered and asked the receptionist to get the GP to fill one out for me to collect. So when I went to get it today, she's put my due date as 16/05/2010 when all my notes say 17/05/2010. Will something slight effect my claim? I know it's my fault for not being with her as she filled it in but hoping someone will tell me it won't be void...please?!

It will be fine. My midwife got my birthdate wrong and it was fine.

I think you'll be okay. My midwife put the wrong year on mine for the due date and I had to appeal HMRC's decision not to pay it, but it was sorted out eventually. If you're particularly worried, can you get them to do the form again?

This happened to me too - the midwife put my original due date on my form when the updated scan date was actually about 3 days later.

It had absolutely no effect on the payment, which I received after only a couple of weeks (despite Christmas and the snow). In fact, I didn't even notice that the date was wrong until I was filing the copy of the form a few weeks later!

If you're worried, get your GP to fill out another one although I'm sure it won't make any difference.

Thanks, I've put it in the post now. Flicking through my notes my due date is written half the time as 16/05 and other times as 17/05, I'm guessing one was LMP and one was dated by the scan, so hopefully the form will be fine otherwise I'll just show them the inconsistancies in my notes. Thanks for your replies.

I wouldn't worry. I got my own name wrong on mine and they still paid it, no questions asked
