Mw said they have no maternity allowance forms so need to download one - anyone know where from please?

I can't find anywhere to download and print off a maternity allowance application form, does anybody know where they are?

At another appointment they did give me a small certificate but I don't think that's the maternity allowance form is it? I'm confused.
