Sorry to be flakey - I've done this x2 in the last few years yet my brain has gone blank - and looking on the internet just confused me!!
I'm self-employed and know that I am entitled to some maternity benefit - but can't remember how to go about getting it!
does anyone know what form I need/ who to contact etc?
