MATB1 form - going to be late giving it to employer

I completely forgot about my MATB1 form at my lasy appointment, so called the MW in a panic the other day. She left one at the GP reception for me, and I have collected it. However, according to my scan I am due on the 12th of September, which makes me 24+3, just in time to submit it before I'm 25 weeks. However, the date she's put on the form is the 5th - which was my date by my LMP, which has been overruled by the scan. So according to that I am 25+3 - so late to submit my form.
Does that matter? Or is it up to HR? Or should I call the MW tomorrow and ask for a correct one?

didn't want this to go unanswered. I used to deal with maternity leave in HR of my local council and although we said we needed the MATB1 for 15 weeks before due date if we received it a few days to a week late it wouldn't matter as there was still plenty of time to process everything. I would let your HR dept know it is going to be late, if it was me receiving the MATB1 i personnally wouldn't have a problem accepting it.

Only downside to the due date on the form is that your HR dept will use this date for calculating your leave entitlement so you will need to start your mat leave at the latest the sunday before the 5th rather than the sunday before the 12th sept if you know what i mean...hope this makes sense x

I submitted mine a few weeks late (passed it to my boss who went on his hols and didn't pass it to HR/Payroll). I panicked but they were fine.

Call them, they'll probably be fine about it.

Thanks both of you, doesn't sound like a big deal, and I will call tomorrow and double check.
laura, yes I know, that's the one thing that's really bothering me. On the form it says "due the week beginning the 5th" (whereas my scan said the 12th). I have checked with HR though and we don't have to take our leave on a sunday (used to, last time) so do you think they'd let me say the 11th instead? Don't think it matters too much as I can carry holiday forward.

you'll need a new one if you want to start leave as of the 11th.

mat leave now starts at the latest the day before you are due (now 4th in your case) or if you deliver early the day after delivery. no more of this sunday business.

but twinkly, the form says "in the week beginning the 5th" -that week includes the 11th so that's why I'm wondering.

I submitted my form a good many weeks late, however i had told my boss, HR & everyone that I was pregnant so it didnt seem to make to big an issue.
