Have lost mine (hid it when I was first pregnant and didn't want visitors to see it lying around and guess before I was 12 weeks but now have no idea where i put it!) and really need to see a dentist. Does anyone know if there's a number I can call to try and get a replacement? Many Thanks!

No idea off hand but mine have never asked to see it.

On the back of my card it only has an address which is

PO Box 854
Newcastle Upon Tyne
NE99 2DE

Not sure if that helps but you could write to them and see.

Or you could call the Department of Health on 020 7210 4850 to ask for advise

The number for the department who deal with them is 0845 601 8076
I have had to ring them several time as mine has not turned up yet. They are very helpful and friendly

In the meantime you can get prescriptions and then claim the cost back - by asking for a form when you pay. Might be worth checking with the dentist if this applies to them too?

Sorry to hijack - does anyone know how long they are taking to arrive? I sent my application off before Christmas and just realised I still haven't got it. Can't remember it taking this long with DS1

mine took about a week from dropping form into gp to arrive.
