Is there a web linkwhere I could print MAT B1 form or is it only provided by the GP and/or midwives?

You need to get it from your GP/MW as they have to sign it to enable you to get maternity benefit.s

So, there is no way I can download the form and take it to the doctor or midwife to sign.

Wonder why there are so many forms that are ok to download and others no

For the love of God, I coulnd't find it on any goverment website.
