I have a dental appointment in a few weeks and the receptionist said I just need to take my exemption form in. Erm...I don't think I have one?! Do I need to get something from my midwife or GP?

You should have a little plastic card that entitles you to free prescriptions and dentistry. This all gets organised by your gp/mw and comes in the post!

I had to send mine off, mw gave me the form in my bounty pack at my booking appt.

My Gp didnt sort it out for me either, i didnt know i needed it was just ticking boxes on back of prescription!! Opps..
When i found out i needed one receptionist sorted it out for me, didnt take long to come-just over a wk i was 20wk ish....

Ta - will ring my GP - actually I'll check first I haven't got a form to fill in, but don't think I have.
