Low PAPP-A, high Dopplers and small baby at 36 weeks

Hi everyone,
Looking for some experiences or advice from anyone who has dealt with low PAPP-A, a small/fetal growth restricted baby, and elevated Doppler pressures.
Here is where I'm at:

  • Background: Low PAPP-A was flagged earlier in pregnancy, and baby is measuring quite small.
  • Monitoring: I've been doing 3 scans/tests a week. My Dopplers have shown high pressure/resistance, but the results have been completely stable for 3 weeks straight now (no worsening, forward flow is maintained).
  • Current Gestation: officially 36 weeks today!
  • The Dilemma: Early on (around week 33), consultants were pushing heavily for an elective C-section due to the high Doppler pressure. But now that we've made it safely to 36 weeks with stable scans, trying to figure out what my realistic birth options are.
i was really hoping for a natural birth if it's safe for baby, but I know an induction (or stick with a C-section) might be recommended given the high Doppler resistance. For those who had low PAPP-A, high Doppler pressure, or a small baby near full term:
  1. Did your team allow you to try an induction/vaginal birth, or did you go straight to a planned C-section?
  2. If you were induced with high Dopplers, how did your baby handle contractions/labor monitoring?
  3. What questions should I be asking my consultant at my next scan to help make this decision?

in my 32 / 33 week scans they really pushed elective section on me, but now 3 weeks on, I feel like I should explore induction maybe?

How big is baby measuring? We had FGR diagnosed at 20 weeks- the plan was to try and make it to 37 weeks which we did our dopplers were high resistance but generally stable but we had a few weeks of brain sparing evidence in the brain readings which made me panic loads - by 37 weeks we had dropped below 1st centile and whilst consultant never would have forced me I was told planned c section was safest option- our boy was so small and we’d had lots of episodes of reduced movements decided didn’t want to risk stressing him with labour and the result being an emergency c section. He was born 37 weeks 4 pounds 2 and although some health challenges in first few days all was okay. It was the best decision. Don’t risk it c section is safer for these special tiny babies

Never had any issues with doppler but my first I had low pappa-a. Measured fine throughout but they were very adamant on not letting me go over due date, delivered naturally few days after following and induction.
Had low pappa-a again with my second (which apparently is rare?) baby measured small throughout and again kept pushing me for an early induction. Baby fell off centiles around 35.5 weeks, but arrived prematurely a few days later after my waters went, again naturally. Did spend a few days in NICU and was tiny and still small over a year later but perfectly healthy.
I was adamant I wasn’t having an early induction second time around after awful first experience and was planning to just ask for extra monitoring.
