Hyperemesis & Pelvic pin advice needed!

Hi everyone, would just like some advice.
Second pregnancy and not been at all easy! I suffered badly with hyperemesis until 20 weeks, still have it but much milder / on and off now. During this time I had some time off sick followed by a period where I had a fit note stating I needed to work from home.
I've since returned to the office once prior to a holiday to Greece (pre booked and non cancellable) which I've just got back from. The hyperemesis is still up and down but during the trip I developed what I think is siactica / pelvic girdle pain, seems from the flight. Now I have really bad pain in my tailbone and the right side of my back, struggling to sit down comfortably and constantly uncomfortable and unable to sleep.
Would like to have advice on how to tackle it with work. I'm meant to do 2 days a week in the office which I'm dreading. But equally don't want them to think I'm taking the piss by swanning off on holiday and coming back and working from home again... I feel super guilty and like they'll think so badly of me. I also don't drive so commuting on public transport and walking up a steep hill from the office is what I'm really dreading..

If you have a prenatal physio close to you I highly recommend making an apt with them. I had sciatica with my 1st and although it didn’t disappear I definitively got to a point where I could work comfortably
