Wrong pregancy notes!

Hi all,
So my Daughter is pg with her first child. She's 28 +3 today. Due 13th Jan.

I accompanied her to her 28 wk appt (community) yesterday. When we got there, her midwife was a bit stressed as she'd been having issues with the computer system that morning.

Anyway, the appt went as normal, fundal height measured etc. Just chatting away as we do and having a giggle and the midwife asked when she was due. Daughter replied 13th Jan. Midwife looked at her screen and said no, you're due 17th Jan - it says here that you're 27 +5. So my Daughter was like noooo I'm 28 +2.

We were all sat there really confused. We were thinking had they revised her EDD when she went for her 20wk scan but not mentioned anything at the scan (it can happen if dates are dating, I know my dates changed about 3 times with one of mine) but she can remember the sonographer saying she was bang on 20 week and EDD still 13th Jan.

Anyway we sat still chatting and I see my Daughter trying to mouth something to me. She was sat next to the midwife and could see the screen. I'm not a great lip reader and couldn't see the screen anyway from where I was sat. They then spoke about her having her whopping cough and RSV jabs and to book in downstairs, then we left.

When we got out, she said Mum there was another woman's name across the top of the screen. That's what I was mouthing to you. It said Lisa somebody. I told her to log on to her Badger Notes (pregancy notes that she has access to) and see if anything had been changed on there. Nothing. EDD still 13th Jan. She also got her growth charts yesterday - been on Badger Notes, nothings been recorded on that even though the midwife showed her on her screen that it had. No blood results, nothing.

Seems obvious the midwife was in the wrong patient record.

I feel like she should have said something at the time! Now another woman will have incorrect notes and they won’t know who it is! In addition to the obvious problem of your daughter’s notes not being recorded. Why didn’t she pipe up?

Why was she mouthing stuff at you?

It would've been so much easier to point to the screen and ask the midwife who Lisa was?

Especially as you were all having such nice chatty conversations.

Sorry, I pressed post before I'd finished typing!
My Daughters name was across the top of the screen at first but for some reason, after she'd inputted her growth chart, it flicked to someone else's notes which is what Daughter noticed just before we left. That's when she told her her due date was 17th etc. ----
We went to main reception to book her vax and mentioned it to staff on reception, they told Daughter to go back up but then the next lady was buzzed to go up. Been trying to get hold of midwife today, no answer. Spoke to community hub who are trying to contact her too and antenatal clinic who are now looking into it.
According to them, nothings been recorded on her badger notes for months apart from the odd one or 2 things.

She was adamant that was her EDD. Apparently that systems a nightmare though according to antenatal, got a mind of it's own.

She said she felt bad because she didn't want to feel like she was questioning her ability but her name was there then next it was someone else's

it's really easy to open the wrong notes on a lot of computer systems and humans are only human. The sensible thing would have been to mention it during the appointment rather than trying mime to you.

Time was of the essence here, your daughter should've questioned what she was seeing there and then. It's very hard to understand why she didn't just say something.

Oh come on! How easy would it have been to say 'Oh I can see what the confusion is, that's not my record, that says Lisa X'?

I've not come on here for a verbal bashing. I've come on to ask for advice. I I asked my Daughter the same thing, told her she should have said something there and then. But ehrn her names at the top of the screen one minute but as soon as somethings uploaded onto her notes, it flicks to someone else's file...And it's not like we didn't try and rectify it, because we did. And we've been trying to chase it up since yesterday because we know we can't just leave it. We made several phone calls yesterday and today
I came on here because I know it's happened to others in the past and had been happening for months before anyone realised

There's been a mix up with unit numbers by sounds of it 🤦🏼‍♀️
