Gave my Mat-B form but only the first page, go on mat leave in 2 weeks!

Im worried my SMP will be impacted - I gave my MAT B form to HR a month ago, I go on mat leave in 2 weeks and HR emailed me last week (I was on annual leave) saying I’ve only given the first page and can I provide the reverse side.

My fault, I didn’t even think about the second page.

I threw it away as we’re having work done to the house and I needed to clear stuff out. I have a midwife appt tomorrow I can ask for another one at but I am worried that as it’s now less than 28 days until my mat leave this will impact my SMP??

I told my employer I was pregnant when I was 20 weeks or so.

Any advice??

It should be fine I would think, technically they only need it 21 days before you want to start I’m pretty sure

ill only be able to give the reverse side of the form 17 days before 😅

Ah I see, it may delay the start of your payments then in that case
