So i have filled out both forms for my sure start grant and maternity allowance. Both forms ask for my MatB1 ( no photocopies) but the midwives only supply me one.

i am 34 weeks pregnant and only just been given my MatB1 form . I have been trying to contact universal credit to notify them of my pregnancy since June and i havent heard anything back.

does anyone know what my next steps are?

How have you contacted UC? Did report a change to health on your journal ? Once you notify UC nothing changes unless you have work commitments as they are switched off. Also maternity allowance is deducted £4£ from UC

I’m reported the health change and left a note on my journal when i was 19 weeks

Can you ask for another form? My midwife gave me two in case my partner needed one for paternity leave in addition to the one I had for my maternity allowance.
