Cord and placenta issues

Has anyone ever had a cord or placenta issue picked up on a scan?
With my previous 2 pregnancies, one of my babies was born and then I had retained placenta, the other baby was born with the cord around her neck twice.
Both had been scanned a week before due date and none of these things had been noticed, but clearly must have been present.
Also there have been a lot of breech babies found with nuchal cords too, so are they just unable to see cords properly using ultrasound?

One in three babies has the cord round its neck. It's extremely common and not terribly important.

Midwives generally slip the cord over the baby's head during delivery. If the cord is tight, it is cut just before the delivery of the shoulders.

As for retained placenta, that cannot be predicted prior to labour.
