41 year old trying for second baby

Dear Mums,

I don’t post often, but I’m reaching out for some advice. I’m 41 and have been trying to conceive our second child for the past 6 months. My first was conceived naturally in 2019. Unfortunately, I had a miscarriage in November 2023, and we didn’t actively try again for nearly a year due to various personal reasons.
Both my husband and I recently underwent fertility testing. His results came back normal. My AMH is 2.30 ng/mL, and my AFC is 22, with no thyroid concerns. According to my doctor, these are good numbers for my age.
However, my fertility specialist has recommended starting IVF as soon as possible, mainly due to my age. While I’m open to considering IVF, I was surprised when she dismissed the idea of doing an HSG test to check for any blockages in my fallopian tubes.
I’m now unsure whether I should insist on having the HSG done before proceeding with IVF. Has anyone been in a similar situation or have any thoughts on this?

I may be wrong but I thought the HSG was for IUI and because IVF is done outside you wouldn’t need an HSG?

To my understanding HSG is for checking if follopian tube is not blocked, I was just surprised because without checking those it’s hard to get overall fertility status

You could get a hsg and try for another few months as it can make it easier to conceive. Or you could go to ivf with pgta. I have a dc conceived first cycle in 2020. I got a hsg in 2022 as we had been trying for nearly a year at that stage for dc2 . Got pregnant same cycle as hsg and had a miscarriage, pregnancy was chromosomally abnormal. Had my lo a few weeks ago after doing pgta with ivf. I was 34 when I had the miscarriage and 37 doing ivf. I think at 41 egg quality is the main concern. My egg quality seemingly deteriorated very quickly in my 30s.

Thank you for your detailed response, yes I was thinking of going for HSG and see what happens. I am ready to go ahead with IVF

If you don’t mind can I ask you how many rounds of IVF did you need to conceive?

@mummyfromabroad it only took one cycle for me. I appreciate I was very lucky.we had been trying for almost 2 years after the miscarriage before I did ivf and no hint of any bfp. I had 2 normal embryos out of 5 (the rest were complex abnormal and discarded). I wish I had done the ivf sooner to be honest and if I was 41 I would be doing it sooner rather than later if you can afford to. You might be lucky and conceive without ivf though I guess. But your chances are greater with ivf.
