Good afternoon ladies,

I'm so confused and my head is all over place basically 2 weeks and 3 days ago I had whay I believe was a 3rd miscarriage I had bleeding pain and passed the pregnancy tissue since then my hcg has been rising very very slowly it's currently at 390 I've had 2 ultrasounds first one they said something about 2mm near c section scar but they wasn't sure what it was few days after I started bleeding lightly and it's been on and off since, then I went for a second scan ultrasound was clear nothing to be seen anywhere but hcg was still rising very minimal at this point I've spent 5 days backwards and forwards having blood tests etc they want to give me methotrexate I asked for the wait and see method they've said yes cos I'm fit and well with no complications can hcg rise slightly with retained tissue ? I asked for the D&C the doctor said cos I've had 2 c sections risk of catching the scar, I'm.so tired and drained and worried to what is happening.

Hey, so sorry for what you’re going through

I’ve had 2 ectopic pregnancies and the slow rising hcg / plateauing / bleeding and pain are classic signs of an ectopic - especially if a uterine pregnancy can’t be seen.
The egg has likely implanted in your tube or c-section scar but they can be hard to see if your hcg is not over 1000.
neither of mine were clearly seen as hcg was always under 250.

my first one I also had what I thought was a mc but then the hcg began to rise again.
I waited for weeks hoping it would resolve but in the end I was treated with methotroxicate- I didn’t really have any symptoms with the MTX and it worked well (needle in top of your bum and quite stingy).
They check your bloods I think on day 5 and 7(?) They expect to see a little increase at first but then a decline on the second bloods. Mine did not decline until the third bloods - So don’t panic if that happens! after this though it resolved fairly quickly. Your immune system will be compromised for a few months so take care and no TTC for 3months either.

the second time the ectopic resolved by itself - expectant management.

the other option is surgery but if they can’t clearly see where it is on a scan aren’t usually inclined to do this.

I've had 2 previous miscarriages and the bleeding and tissue I had come out of me 2 weeks ago looked just the same that's what's left me confused spotting I have at moment is on and off no pain etc I have to go for blood again tomorrow I'm definitely thinking methotrexate they can't do surgery on something very tiny and can't see it.. I'm so scared with this whole process just want it over with now, my partner is against me having the injection cos he read stories online about the side affects etc.

I'm just tired 😫....

@Lisaaustin90 people take methotroxicate all the time for autoimmune conditions - this is literally one shot. At the most you may feel a little rough first a few weeks but I was literally just tired. If you can take it off work that would be good. There’s always a risk with anything of side effects.

I’ve had a mc too, I definitely passed lots of clots with the first ectopic but I think it was just my lining not a pregnancy. When I MC at 6 weeks it was MUCH heavier but but I had medical management so that may have been why.

a very very rare possibility is that it’s a herotopic pregnancy where two eggs have been fertilised, one ectopic and one actual pregnancy that has miscarried but the odds are in the thousands.

if you are feeling well and your hcg stays low it’s generally acceptable to expectant management a bit longer. The first time round for me though i just wanted it over and it had been weeks and weeks of limbo.

it’s you that’s got to go through the process so i hope you can make the best decision for you xx

@Xwx1010 I'm definitely thinking got having the methotrexate i was worried about any rupture but I got told by a doctor that my hcg is so low for anything to rupture its a tiny bit thay small it can't be seen on ultrasound but I definitely had a miscarriage so has to be tissue still in there not an ectopic I've been so scared with all this been to 2 different hospitals my local hospital did a scan on 17th April they said something about 2mm was seen near or on c section scar was too small to see they wanted to do a lapascopy to inject something into whay they thought was a sac then not long after I started bleeding lightly so then I went to another hospital had a scan on 23rd April they said my scan is clear nothing in womb tubes ovaries or on c section scar there is just nothing at all..

Does retained tissue cause spotting and slight rise in hcg?

I had so many scans it changed every time between seeing something and not - it’s because it’s so small and it could be anything - fluid, scar tissue, fibroid etc. the scans are just not clear enough when it’s so small.

retained tissue can cause the spotting and if a little bit of the pregnancy is left the cells can try and grow again so hcg still trying to increase which is really sad I think.
may be worth seeing how it goes over the next week or so as it naturally may start to decrease and pass anything remaining. Xx

@Xwx1010 I'm leaving it to see if anything passes or sorts itself out i know what to look out for for any problems..

Each doctor says something different when in fact they just don't know another doctor daid possible ectopic but I tried saying how can it be an ectopic if I had a miscarriage they wouldn't listen to what I was saying they basing it just on hcg little bit rising.

I've been so upset scared and pushed around with all this and no on listens to me this had been ongoing for 7 days my mental health has been up and down. I am now doing the wait and see approach before any medication. Xx

@Lisaaustin90 not to put the fear of god in you but mine went on for over a month. It’s very draining and hence why I opted for the MTX in the end. Pregnancy tissue or ectopic the MTX will stop the hcg rising and the tissue/pregnancy will pass. In the end that’s all that mattered to me - not where it was or what scans were saying. It was desperation for it to be over.
if you can stomach the wait expectant management is of course preferred but MTX might be the plan B if you’re in limbo for ages.

they can’t say 100% ectopic if they can’t see it, hence all the different opinions but it’s very very likely it is this or an early MC. The hcg plateauing, dropping and rising is classic for ectopic.

anyway good luck with it all and I hope it resolves itself soon for you.
