16 week consultant appointment - what to expect?

Can anyone advise of what normally happens at your 1st consultant appointment at 16weeks?

I am part consultant, part midwife led this time due to previous birth complications and my age.
My midwife forgot to schedule any referral appointments for me so the 16 weeks consultant appointment has been rushed and I have no information about it.

What happens at this appointment? Do they perform a scan or anything? Do I need to take a urine sample etc?
Appointment is tomorrow and still not received any info so thought people here may be helpful.

At mine, no scan, usual BP check and urine dip. Then we had a chat about the specific reason why I'd been referred (eg. First pregnancy it was because I'd had fertility treatment, second pregnancy was due to complications that happened during the first pregnancy). She explained my options, made a couple of referrals for me and then said she was happy for me to be midwife led. I didn't see her again until I was 36w and developed preeclampsia.

I'm consultant led for other reasons. They went through the risks during pregnancy, birth and afterwards.
They discussed the support I would have in place and their recommended treatment.
This was in 2020, the first appointment was pre lockdown and I had a second one during lockdown. This was a call where they checked everything was ok.
I had an appointment with my consultant for my condition in the third trimester and they put together a post birth plan for support during the days after birth which was then followed. I needed to be in for 3 days due to medication and they recommended that I had a single room as poor sleep risks me getting acutely unwell and I might need support overnight.

I'm pregnant again and due to previous losses my appointment is a bit later too. I'm 16 weeks and I think my appointment is in may.

Thank you both for your replies, both reassuring and I now know to take a urine sample with me!
My midwife was supposed to refer me for a diabetes check and to fetal medicine for some testing which also hasnt happened. Would this be something the consultant would do or do I have to keep contacting the community midwife for this do you think?
I feel so in the dark this time around and almost feel a bit forgotten about.

I'd speak to the consultant about it. If it's not something they will arrange they will probably put it in your notes asking for the midwife to sort it
