Hi,

Just seeking some clarification please.

My midwife has told me that all I need to do is write my name and address on the Mat1B form.
When I've looked at the other side, neither Part A or Part B have been filled in but there is a valid doctor's stamp in the bottom right-hand corner.

If I submit this form to my employer with just my personal details and the doctor's stamp, is this enough or will it be rejected?

There's no mention of a due date etc.

With mine the midwife wrote due date then signed and stamped it. But my employer did also need the back with my details on it (even though they already had said details). I think your midwife might have got confused?

I'm a midwife. That's not right, she should have filled in the front side (either part A or B depending on if it's before or after birth, and the details about your due date and the date she examined you, plus her NMC PIN). You then fill in the back.
I think she's given you a blank one! I'd take it back to your midwives and ask them to fill it in. She needs to sign it as well.
