Previous Lletz treatment and cervix scans

Hi all!
currently 14 weeks pregnant and I haven’t had a letter or anything to confirm that I need to get my cervix checked due to previous lletz treatment. I had a lletz for CIN1/CIN2 about 7 years ago. I have asked my midwife who is looking into it but just wondering if anyone has experienced similar?

Hey, I had lletz about 3 years before my first pregnancy. They brought me in at about 16 weeks to see a consultant, they measured my cervical length and said they didn't believe I needed a stitch. They checked again at the 20 week scan and then signed me off as not needing one.

This time they seem less bothered as it didn't cause issues last time but I've got a consultant appointment due to pre-eclampsia in labour at 17 weeks so was going to mention it again then.

From looking at the NICE guidelines (can be useful as then you know what is 'supposed' to happen) it says -
"Consider prophylactic cervical cerclage for women when results of a transvaginal ultrasound scan carried out between 16+0 and 24+0 weeks of pregnancy show a cervical length of 25 mm or less, who have had either:

preterm prelabour rupture of membranes (P‑PROM) in a previous pregnancy or

a history of cervical trauma. [2015, amended 2019]" (lletz is given as an example of 'cervical trauma')

Is your 20 week scan booked? I would raise it again with your midwife at your 16 week appointment - sounds like that's the time when they'd do a measurement to see if they needed to take any action but maybe it'll be part of the 20 week scan?

Hey OP, in my unit they only recommend surveillance if 2 or more lletz procedures in the past so maybe it's similar where you are.

On rare occasions a more complex single lletz procedure might have to remove more and in turn shorten the cervix but in that case they'd tell you that they had to perform a 'deeper' lletz. That's the only scenario where you'd need surveillance with only one lletz in the past. Again that's just how it's done where I am, worth asking the recommendation in your own area.

I didnt get mine checked with 1st 2 pregnancies but this time around they said they'd refer to colp for details of how deep it was to see if needs checking. So no i don't think it's checked as standard but run past your midwife for reassurance x

Yes my 20 week scan is already booked and my 16th week appointment with my midwife is actually on my 17th week so I’ll double check with her then! I thought they’d mention something at my dating scan but they didn’t and when I asked they said my midwife should be the one to refer me.

@remaininghopeful23 thank you for commenting! From what I remember I had a choice on the day whether to have the lletz or not or just wait to see if the abnormal cells would disappear on its own. Whilst I was there I just opted in for the lletz so I’m presuming they didn’t remove a lot. On my notes I am classed as ‘intermediate risk of preterm birth’ and not high risk so maybe that’s why? I’ll ask my midwife at my next appointment just to be safe.

Sailed through my first pregnancy no issues whatsoever and went over my due date.
2nd pregnancy had spotting but that was because of Group B Strep not cause of this. Had an induction a few weeks early. No issues with labour or birth x

It feels like alot to me feels like a massive chunk gone. Knocks me sick tbh!! But i have no idea if it's alot in medical terms.
