Should I be concerned-Pre Eclampsia?

I am 32+5 40 IVF and first baby had BP done yesterday with midwife 134/102 which she said was fine (I didn't see the reading until it was inputted in to my app) my BP has typically ranged from 98/65 to 102/70.so seems higher to me? I also have some quite severe rib pain on the right side which I've had for a few days now and it's getting worse (MW said it's ligament pain), also yesterday she said there were traces of protein in my urine but wasn't concerned. At growth scan baby was 12th centile so measuring small but again they weren't concerned just want to monitor in 3 weeks.
I'm not an overly anxious person but it's just flagging a concern with me that there are a few markers of pre eclampsia. I have another midwife appointment in 2 weeks and a growth scan in 3 weeks so I can keep a close eye on symptoms until then or I don't know if I should contact someone sooner (GP or triage?) I do trust the professionals but also know things get missed sometimes and it's better to be safe than sorry..any experiences/thoughts welcome..

Ring triage

Sunshineclouds11 · 02/05/2024 07:44

Agree with triage

PilgorTheGoat · 02/05/2024 07:44

I had pre eclampsia with my twins. I don’t want to worry you but I’m surprised the midwife didn’t comment on your raised BP especially with the protein traces. Do you have a way to contact your midwife team just to discuss it for reassurance? If not I’d probably try to see the GP, even if it’s a case of them saying it’s all fine.

Taq · 02/05/2024 07:49

Go to triage today, do not delay. Tell them what you have said here, that you had a BP of 134/102, proteinuria and epigastric pain.
Do you have any swelling? Headache? Blurred vision or flashing lights? If so tell them that too.

NICE guidelines diagnosis of pre-eclampsia is a new raised BP in pregnancy with proteinuria. Don’t be fobbed off, go today.

Also don’t panic, if it IS pre eclampsia they will monitor and treat. But you need seeing today.

PS don’t bother with the GP, go to triage.

Thanks all I don't have any other symptoms at all and feel ok I'm generally pretty fit and healthy. Have just done my BP at home (have a monitor as ironically had low BP and fainting earlier in pregnancy) and it's 112/70 so seems ok. I've had the right sided pain for a while was more in my back before moving round to below my right boob, it feels muscular as moving around relieves it. It does hurt to touch tho like a bruise almost.. but that could be normal for ligament pain/Rib flare.
I had urine checked Tuesday and it was fine then yesterday just a trace of protein. Baby is active enough and they checked placenta flow at scan which was fine. It's so difficult to know what to do isn't it?

I would still give them a ring.

They'll ask you in for a check or give advise, it's what they're there for.

Go and get checked. I'd had debilitating rib pain, vomiting, slightly raised BP and a small amount of protein in my urine. My midwife booked me to see a consultant a week later. By the time that appointment came around I'd already had an emergency c section due to preeclampsia. Looking back I can't believe she didn't do more, and I was a naive FTM who trusted her. The outcome could have been so different if I hadn't called triage a few days later (they tested me for PE and then they rushed me straight into theatre).

JRTfan · 02/05/2024 11:31

Thank you all I'm feeling absolutely fine at the moment and baby is kicking away will take my blood pressure again today and keep an eye on symptoms, if anything arises I will call straight away. Rib pain has eased completely now so think it is more muscular from position I lie in. I've had no sickness, blurred vision or headache.

I had pre eclampsia and never had vision issues, headache, nausea, nothing except high bp, protein and swelling that was passed off many times as normal pregnancy swelling. Just keep an eye on your bp at home and if it gets higher get checked out even if you feel fine.

Same.

I'm a midwife OP and I urge you to go to triage.

You can have severe PE and have no obvious symptoms.

You need blood tests. I'm shocked that your midwife thought that a diastolic of 102 was of no concern. Especially with a trace of protein in your urine.

I've just checked the NICE guideline to confirm what I was going to say just to make sure it was definitely accurate.

You can look at the guideline if you want OP, it may reassure you. The blood pressure is only considered high if it's over 140/90 according to the guideline. So that's why your midwife said it was fine, it is according to the guideline.

Likewise, the guideline only recognises protein in the urine if the reading is at least +1. So again she was right that your urine as trace protein is fine. This is because trace protein can often be caused by contamination such as with vaginal discharge. I'm early in my pregnancy and have trace protein at both my two urine tests so far.

All that being said, I can appreciate the blood pressure is higher than usual for you, and you have the rib pain. So if you're concerned, do go ahead and ring triage anyway, as you say it's always better to raise it even if they just say what I just did above and tell you to continue monitoring closely.

All the best OP.

EDITED: So sorry OP, I've just re-read your quoted post and can see your bottom number IS over 90! Definitely ring triage!

Killerqueenie · 02/05/2024 13:26

Definitely ask to get checked further. I had so many symptoms of pre-eclampsia except for protein in my urine or a high BP. I had swollen legs and feet and hands, rib pain, and kept seeing stars. I went to triage multiple times, and they said, "Your BP is fine, but we might do a blood test" (they never did the blood test) then sent me home. When I went for a routine appointment a week later, my blood pressure was 220/110. I was immediately induced and ended up having an emergency c-section due to almost having eclampsia, and my liver was failing. If I had had a blood test done when I got checked at triage, it would have shown problems with my liver.

I dont want to scare you, but It's so important to get checked, op. I had been feeling relatively OK the day that my blood pressure was that high and wouldn't have known otherwise. I dread to think what would have happened to me and my baby if we hadn't been seen that day.

anicecuppateaa · 02/05/2024 13:29

I’ve had pre eclampsia twice, and both times was admitted to hospital until babies were born. I had no symptoms apart from raised bp and feeling off. Please get checked just in case.

ProjectKettle · 02/05/2024 13:49

Dd2 is 4weeks old and i had preeclampsia with her. My urine only showed as trace but it was sent away for analysis as my BP was very high (160s / 115s range) and the actual number that came back was way over the threshold for PE. As with everyone above, please ring triage and get checked out. I def wouldnt be waiting two weeks. I ended up being told to go into the assessment unit every 3 days for BP profiles, medication reviews and monitoring etc. But we got DD here safely which is all that matters.

Re checking your BP at home, my MW didn't really like me doing this as my machine was just a basic one from a local pharmacy and gave off significantly different numbers to when it was taken by the MW. Maybe when you speak to her you could check on what your hospital likes you to do?

JRTfan · 02/05/2024 15:10

Thank you my BP this morning was 112/70 which I think is OK surely it would continue to be raised if there was an issue. I will test again when I'm home from work. The urine sample was from early in the day which apparently is more likely to show protein..and rib pain has gone. I really do appreciate the responses and understand that it's important to get checked but think the pain/tiredness got to me this morning. Pretty sure it's a ligament/muscle issue when I'm in one position for too long as was agony when I woke up..
