Letter of intent for maternity leave

This will be my second maternity leave, but everything I did first time round has slipped my mind.

I'm currently 25 weeks pregnant and I'm due June 20th and would like my last day in work to be may 24th. I've handed my MATB1 form into my work but I've heard nothing back yet. But when do I write my letter of intent for my maternity leave? And what do I write in it? I really can't remember what I did last time round. I work for the same company still, but in a different store

Do you have a copy of the parental leave policy/staff handbook etc? That should tell you what you need to say.

My company's policy literally just states that you need to tell them you're pregnant, the due date, when you want to start leave and confirm that you're planning to return to work afterwards, so that's all I said in my email

We have an app with information on but it doesn't say anything like that. Doesn't say if to send it to a address or just hand it to my line manager. So I'm really baffled
