[5 weeks] Low HCG + can't find baby in Ultrasound + spotting

Hi,

New to this forum.

We started trying for a baby and soon we found that I had missed my period and pregnant test was positive.

Yesterday I was 4 weeks + 6 days pregnant. I noticed some spotting - about 6-8 drops. I have also been having occasional cramps, but they were light-moderate (as compared to my period cramps). I & my husband panicked and went to the emergency care over night. We had blood work done (for the first time since we figured out we were pregnant), and got both transabdomen & transvaginal ultrasound done today morning (5 weeks pregnant).

My HCG at 4 weeks + 6 days was just 112, and both the ultrasound didn't detect any baby / embryo / egg.

The ER doctor concluded that we had a nonviable pregnancy & that probably the egg has already passed.
We are asked to get another HCG test done 48 hrs later. I haven't had any more bleeding since yesterday evening.

We will be getting the tests done and meeting our primary care doctor next week, but we are trying to read about this and are acertain about multiple things.

Hoping to get some insights from others who have been through similar experience.

Based on what we have read so far, it seems that it could be one of the following things:

  1. A miscarriage has occurred (most likely)
  2. It's very early in the pregnancy to detect the baby in Ultrasound & HCG being low, and there might still be a slight chance at a healthy pregnancy (some other cases we read on this forum suggested this).
  3. It could be a ectopic pregnancy.

Not looking for an exact / accurate answer, and we will be following up with our primary care on the next steps based on the follow-up tests we do, but just looking for some support & insights.

Also, additional question: If this has actually resulted in a miscarriage, when will be the next period & when can we try again?

Any insights / support is much appreciated.

Hi OP

So sorry you’re going through this stress. At this stage it really is a waiting game I’m afraid. You are very early in your pregnancy and I don’t think you would see anything on a scan yet especially with a HCG of 112. I really hope you get a positive outcome. All I can do is tell you my story.

We started trying for our second child in January this year. I got faint positives in both January and February but my period turned up a few days later just after 4 weeks. Then in March I got strong positives but again this turned into a chemical around 5 weeks (tests got lighter and I started to bleed).

I then got another positive in June. From my previous experiences I was anxious and didn’t feel like my tests where getting any darker so was referred for a pelvic ultrasound at 4 weeks 5 days, no signs of pregnancy was seen but I was assured it was super early and not expected. The next day I went for a HCG test which came back at 129. I had another 48 hours later which was 337 so had more than doubled. My GP was happy but I was still very anxious. A day or so later I then started spotting so called the GP again as I thought it could possibly be ectopic and was referred to the EPU for a scan in a few days tome. They did see a sac and a yolk sac in the right place at this scan but measuring small for my dates (should have been 5 weeks 5 days but was measuring 5 weeks). I was booked in for another scan in 10 days. I wasn’t optimistic especially as continued to spot. However at my next scan they saw and embryo with a heart beat and it had caught up a few days but still measuring a bit small.

Thankfully at my 12 week scan baby was measuring exactly where she should be and I’m now 28 weeks.

I know the waiting game is really hard. My HCG was also on the lower end. Your next HCG result should give you an indication of how things are progressing but have everything crossed for you. Good luck x
