Low papp a

Hey all,

I had a consultant appointment today due to a previous MMC in January. He said my down syndrome results came back very low and no concern but I do have low papp a... He doesn't recommend aspirin at this stage as I'm past the point of my previous loss (I'm 14 weeks) and have also had very very mild spotting when I wipe, which we believe to be caused by thrush as it really seems to be external blood when I wipe only.

Anyone have any knowledge on low papp a and the risks? Google is scary so I prefer not to Google it and everyone seems to be taking low dose aspirin for it so concerned I'm not?

Midwives would suggest aspirin when there’s low Papp A.

I'm 17 weeks and currently on aspirin for the duration of my pregnancy for Papp A. Can you get a second opinion?

Ttcmumma · 10/10/2023 12:36

How strange... He mentioned the aspirin but said I don't need it. Do you mind me asking how low your papp a is? @FortheBeautyoftheEarth

@Ttcmumma My maternity record just states that it is <0.4 MoM

Ttcmumma · 10/10/2023 13:13

@FortheBeautyoftheEarth ah mine is 0.78 MoM... Maybe I'm only borderline low and that's why it's a different decision. Will try find a second opinion but not sure who to contact

@Ttcmumma Papp-a is only considered low if it's less than 0.4MoM so not sure why you've been told yours is low

TTCbaby2023 · 10/10/2023 14:15

0.78 MoM is not low Papp-A. Low would be below 0.4. I have 0.8 MoM and was told everything was fine. I am still on aspirin though but it is more down to my age as a precaution.

🤔 he specifically told me that it is low and the rest of the results are normal but I will need extra scans. Being booked in for 32 and 36 week growth scans. He contemplated the aspirin for about 5 minutes which is what worried me when he decided to go with the decision of not giving it to me. Might need to change consultant if he's going to be this confusing!

Thanks tho ladies, it's reassuring to know that it's not actually low and that makes me not worried about the spotting as I was almost certain it was from the thrush irritation until he scared me today!!

As others have said your Papp A isn’t low. It’s not going to hurt taking the aspirin.

Im on aspirin for different reasons.

Thankyouthankyoujellybean · 10/10/2023 21:54

Just popping in to say that I had low papp-A with DS2 and was never put on aspirin. I don't know why, but he was average weight and very healthy.

Ttcmumma · 24/10/2023 22:00

Had my last consultant led appointment today as they're happy to discharge me to usual midwives and the doctor above the consultant has no idea why he mentioned low papp a or the need for aspirin as there are no concerns with my results!

Relieved for you but to be fair not surprised. Your levels seemed absolutely fine.

redxlondon · 25/10/2023 18:05

There’s some incorrect advice on here so definitely speak to your doctor. They will help explain why a low PAPP-A does not always indicate aspirin should be taken.

Hi ladies,

Have been following threads on low papp-a for a while and genuinely looking for some thoughts / views experiences on this.

Had my combined nuchal scan at 11weeks. Result came back immediately with low papp-a (0.1 MoM) and head (BPD) abnormally large compared to the body (CRL). The head is 95th+ percentile and body 55th percentile. The risk for the trisomies were incredibly high, but NiPT came back low risk.

i’m not sure what to do. I understand i can do amnio to know for sure if the baby has any trisomy but I am concerned that even with those results coming back fine the risk of 2nd trimester miscarriage, still birth and pre term birth are incredibly high. Has anyone been in this situation before?
