10 weeks pregnant - possible miscarriage, haematoma or something else?

I'm thoroughly confused with what is going on with me at the moment. Sorry if this is TMI! This is my first pregnancy so I don't really have a clue what's going on.

I'm 10 weeks pregnant and over the last few days I've been having intermittent period like cramps, lower back pain and bleeding. I've been feeling a bit on the crap side since the start really (tired, bit of nausea, sore boobs etc) but started feeling more like my normal self on Xmas eve - was thinking it might be because the pregnancy is progressing, but now I'm wondering if it's a miscarriage, which is why the pregnancy symptoms have gone?

On Monday evening I had two instances of bleeding - not much initially, but on the second occasion the blood was fresh red and pretty much dripping out with the results in the loo being pretty much red. Tuesday morning there was some oldish blood, had more fresh bleeding Tuesday night and then some older blood again Wednesday morning. I've also been passing stringy bloody mucous clot like things intermittently too.

I had quite bad period like cramps and back pain on Wednesday night that kept me awake and had a bit more fresh blood at the same time. Had more blood this morning too but not much during the day, though it appears to be starting with a fresh stringy blood clot again now evening has come around.

Weirdly the cramps and bleeding seem to happen mostly in the evening / at night - I've had light cramps, back pain and a little bit of bleeding during the day, but not anything as bad as at night, and in all cases the blood has only come out when I'm going to the loo - nothing much on any of the pads I've used.

I managed to get a doctors appointment today who has referred me for a scan at the EPU on Monday and he seemed a bit bemused as to what was going on due to the inconsistency of the bleeding etc, and he thought if it was a miscarriage the bleeding would have progressed quicker and there would been more, or at least more often than I've had.

I'm thoroughly confused as to whether it could be a miscarriage or a haematoma or something else, nothing I've managed to find has documented quite the same thing in that the bleeding just seems to be really happening at night.

Anyone have any similar experiences?

I’ve had similar in both my pregnancies. Light bleeding & red/brown discharge on the tissue when wiping.
It’s not a crazy idea to take a photo if you want to show medical professionals.

My first pregnancy, I had light bleeding and blood on the tissue maybe 3 times, all in the first trimester.

My current pregnancy, I’ve had similar and scans showed no issue, just a low placenta.
2 weeks ago I had a lot of bleeding and just randomly in the afternoon - filled 3 pads (when I called the hospital, medical staff asked how many pads I filled and how quickly).
I was seen nearly straight away as I was 15 weeks. Nothing could be seen on scans, everything looked fine. Everyday since, I’ve had light bleeding and red/brown discharge when I go to the toilet. I was scanned yesterday and they saw a haemotoma which they will monitor weekly for now.
I was told not do anything for now - light walking in the garden for fresh air, and otherwise just sit on the sofa. I can’t pick up my daughter and I’m to stay calm. And definitely no sex.
Interestingly, we did have sex the night before I bled a lot. But my doctor said that’s unlikely to be the cause. I am nearly 40 though, so I think that makes me more prone to complications. And I’ve experienced some traumatic family issues in the last few weeks - sobbing and hyperventilating etc.

Sorry if I’m rambling, but it’s not uncommon at all. Just take it really easy and rest as much as you can until you know more.

I came off contraception in September had a period every 4 weeks and last period started 21/12/23. Lasted 5 days.. January I didn't have a period. Tested on 21st which was negative. Tested on 24th which was very faint. Tested on 28th was a positive with darker line. I started to bleed on 27th Feb just spotting so wasnt too worried. Went to EPU and had a scan on 3rd March. The lady doing the ultrasound couldn't see so did an internal one. She said are my dates wrong at all and I was so sure due to my last period date I would be about 10 weeks. (I know your not actually pregnant the first two weeks or so but they usually go by when you last had a period) they advised me it looks like a missed misscarriage because I'm showing only 6 weeks. I then went to another room really upset and had to wait for a report. After I went to another room to give the report to the midwife who asked if my dates could be wrong. I said well I had a negative test in January then explained my periods were usually irregular but we're every 4 weeks since I came off contraception. She said they may not be regular and I may have ovulated mid January and just before coming on actually fell pregnant. She then told me I am having twins which was a massive shock. Then said I have a sub chorionic hematoma next to the sac but that it's not harmful to the pregnancy due to being next to the sac not above it? She also said it looks like a healthy start to a viable pregnancy and that due to the Sub chorionic hematoma (SCH) I will continue to bleed and may have clots and be heavy with cramps too. I said my pregnancy symptoms such as sore breast and tiredness was extreme a few weeks ago and is starting to fade and she said that could be normal aswell. She wanted to be positive and said as of now I am still pregnant and all is well but I then went home and started to bleed a lot more and was up all night with severe cramping. I didn't go to A&E as I could manage it but I didn't sleep at all. Early hours of the morning I passed a huge ball like clot and then since then have continued to bleed a light flow and have some cramping. I am so confused as to whether I just had a misscarriage or it's the SCH. I have to go back next week to see if the pregnancy has progressed. They said in the last scan it was too early to see heart beats but there was two yolks and a sac. I am super nervous and unsure what to think. Other than bledding and cramping I have no sore boobs anymore. I never had nausea with this pregnancy and I thought with twins it would be worse. My tiredness has improved and the only things I notice that are different is constant eye twitching, itchy nipples and pain in my lower back and thighs.

I also had my cramping and bledding mostly at night too. I'm not sure if we're experiencing the same thing but until my scan I won't know if the pregnancy is progressing so bring in limbo ATM sucks!

How did it go for you? Any updates?
