HCG not doubling / slow rising at 5-6 weeks pregnant

Hi all,

Last week I experienced some concerning symptoms (brown spotting, stringy clots, and cramping) for which I was seen at EPU.

My GP ordered some HCG and Progesterone bloods on Thursday 10/11/22 which came back at 9,358 iu/L and 29 nmol/L (equiv to 9 ng/ml, I am from the UK). On Friday 11/11/22 I had a transvaginal ultrasound which measured me at approximately 5-6 weeks pregnant. No heartbeat could be seen so I am due a follow up scan on Friday 25/11/22. As I was concerned about my Progesterone level with having PCOS, a private Obstetrician gave me a private prescription for Cyclogest pessaries to go on with.

In the meantime, the spotting has stopped but I am still experiencing cramping. I have had another set of bloods taken on Monday 14/11/22 and I am a bit concerned as my HCG doesn’t seem to be doubling every 48 hours which my GP said is required for a successful pregnancy. My HCG has gone up to 22,391 iu/L and Progesterone is now 62 nmol/L. My concern is that my HCG has only just doubled over about 4 days (9,358 to 22,391). Is this rise slower than expected for a viable pregnancy at this stage? I’m worried if it could indicate a miscarriage to follow?

I think over 3-5000 hcg is only meant to double about every 72 hours. It slows down the higher you get. After about 5000hcg the only accurate measure of viability is a scan as you would expect to see yolk sac by that point and heartbeat after 20k bhcg.
can you try and get the scan booked in sooner - they should be able to see a lot more with hcg at the levels you have now or get a private scan? Good luck OP

Hcg slows down hugely when you get into the thousands. Those are big numbers you have (although you haven’t said how many weeks you are?)
There’s no purpose testing hcg after about 5000 as a heartbeat can be seen on an ultrasound - I’d suggest booking in for a scan if you’re worried about viability of the pregnancy. best of luck

Hi thank you for you message I am supposed to be around 6 weeks today i am going for a scan as this has worried us… it’s not always certain to see a HB past 5000hcg… but we shall see what happens with the results today. Thank you xx

@Texasholdem20 we have just been to the scan we are 6 weeks and we seen our little babes heartbeat !!!! …. So we are feeling a lot better! We do get more bloods results back later today. But that is the reassurance we needed for now. Thank you so much for your message :-)
