Bleeding 5 weeks on progesterone pessary

Hi everyone,

Long term lurker first time poster. I’m looking for some hope I guess. A bit of history; MC Sept 21 ending in D&C, I was 11 weeks scan showed baby hadn’t grown past 8 weeks 4. Ectopic rupture Jan 22 at 5 weeks 3 days. Chemical April 22. Had all recurrent miscarriage investigations done and just low Vit D. Now pregnant again 5+1 on Utrogeston pessaries, started spotting on Wednesday 20 July and had a rainbow of colours from pink, brown, and pinky red and no pain. Very intermittent, I’m looking to see if there is a pattern of every 12 hours ish after a pessary I bleed, I’m wondering if the are irritating my cervix? I’ve had HCG done at two separate hospitals rise of 116% in 48 hours and second hospital 89% in 48 hours Dr was keen to ensure I complete tests at both due to lab differences. Don’t know to prepare myself for the worst or is it there any hope obviously given my history it’s terrifying and I’m going from hope to tears. Has anyone else has irritation or bleeds with the pessaries? Apologies for the longest post in the world!

I wss on progesterone this pregnancy 17 weeks due to 4 miscarriages, but I put them up the back end as I was scared of bleeding can you use yours that way.

Hi @Babyghirl thanks so much for your reply. Lovely to hear you’re 17 weeks too hope it’s all going well. I’ve read about that but unfortunately mine are vaginal only 😞.

You can Take utrogeston orally even if the ones you have say vaginal. I had spotting from vaginal use and everything was fine.

I'd see if you can switch to cyclogest (they're vaginal or anal). I think vaginal ones can irritate the cervix and cause bleeding.

@LilacPoppy thank you for your reply, that is really interesting, didn’t know that! Thank you for sharing that’s made me feel a little less panicked (for the next 30 mins anyway). Interestingly I’ve just spoken to my Mum they’ve put her on the oral Utrogeston tablets for HRT and she’s just started spotting! So hopefully all will be well with this little peanut ❤️ thank you!

@seven201 thank you for your reply. When I speak to them tomorrow about our plan for the next week. I’ll ask about swapping as it seems I’m not alone in this! I can feel it’s sore down there (sorry TMI) so wouldn’t surprise me if it’s irritated. Thank you!

Just to tie up the thread for anyone in the future. At 5+6 I’d had a little more bleeding and had some pain which came and went, my intuition said push for another scan. At HCG of 2700 there was no sac in my uterus and a mass near my ovary in my remaining tube measuring 1x1x1, although no sac there either. With no baby in sight we had to make the tough decision with a medical team whether to wait it out or be treated with a Methotrexate. As I only have one tube we have decided on Methotrexate and hoping this works, I didn’t want to risk my tube again and as no baby in sight it feels like the safest option. Sorry this isn’t a more positive story, the spotting could still have been the pessaries just I had something else niggling. Now just trying to get through the next few months then think about further investigation on my tube and possibly IVF.

I am so sorry to read of your outcome. I had so hoped for it to be positive but thank you for tying up the thread. At 5+4 following IVF and an embryo transfer, I started bleeding Saturday. I am awaiting my second BETA on Wednesday to confirm what my hcg levels are doing but feeling really pessimistic.
I hope you recover quickly both physically and emotionally ❤️

Hi @Jazper I’m so sorry to hear you’re bleeding it is terrifying. Thank you for your kind words too. I just don’t know what to do anymore think we will give it a break for a while to try and recover, and they may suggest IVF for us now too (got some weight to shift for that). I hope your HCG comes back positive and everything works out for you. If something doesn’t feel right though push for a scan, any slight twinges one side, any dull aching, anything. My first ectopic I was lead by the hospital team as we were so scared, in hindsight they were slow to react, looking at my treatment this time (different hospital) they’ve been quick to act. My current hospital are actually horrified how slow the one in January were and have suggest I report them. So I’ve come to the conclusion I probably wouldn’t have lost a tube and been so poorly if I had been at my current hospital back in January (I got sent there out of hours as we live between the two wish I walked out and went to where I am now) 😞. Time is critical for safety and give you more options. Keeping everything crossed for you, not everyone has the run of bad luck like me, there are much more positive stories out there than me xxxx
