I had an early scan at 4wks 3 days (due to a history of ectopic pregnancies).
My HCG was 2874 (18-20 DPO max- which I understand is on the higher side)
Looking at the scan this evening, I cannot unsee a second less dark circle to the left of the more apparent circle. Am I going crazy? Does this look like multiples?
It wasn't mentioned at all in the scan (tho most of the scan was focusing elsewhere) which makes me think I'm just imagining things.
Slightly freaking out nonetheless
Does this look like twins?

congrats! I think that the sonographer would have said if they thought you were pregnant with multiples; did they confirm that they saw a gestational sac etc? My early scans had little patches that looked like they could be less clear sacs, but it was a single pregnancy. Perhaps could be just because ultrasound images aren’t super clear. Best of luck with your pregnancy x

Congratulations. It could be the yolk sac forming? It’s really hard to tell so early on.

One in eight pregnancies begins as twins, I believe, with only 1/80 births being twins. Basically it's very very common for a pregnancy to start as twins in the very early stages before you even know what's going on.
