Causes of an unhealthy and possibly failing placenta?

Not really sure where to post this..
I recently gave birth to DC2. Both have been low risk pregnancies.

I had a c section 2 weeks early as a growth scan showed that he had dropped quite a lot down the chart since the last growth scan - blood flow from the placenta on scans was normal.

Afterwards, the midwife told me that my placenta did not look healthy and was showing signs of failing - it was in pieces when when they took it out. I asked the causes of this and she said smoking/alcohol (none of which I did) or just one of those things. My first pregnancy the placenta was fine.

It's played on my mind since - I'm very thankful that I had growth scans but think what if I hadn't..? Has anyone else had an unhealthy placenta or have any ideas what caused this?
