I have an app called Nurturey Pink Book. It helps me track every single thing about my baby. From her height and weight to vaccinations. A friend recommended it to me when I was pregnant. It even helps me track my pregnancy cycle and make appointments with my GP!!
I hope that helps! :)

When my dc were born (they are 13 and 10 now), we got a “red book”, which is essentially a small ring binder which has pages/charts/proformas to record all this information. It was filled in by the MW, nurse, health visitor etc every time my dc were weighted, vaccinated etc. I keep things trike eye test results, dates of Covid vaccines etc in there too.

Red book. Health visitor should also ask when/if they're around if you've booked them. My GP surgery also sent me letters when the next round of vaccines were due. I also booked in the next set (12 + 16 week) vaccinations while I was at the GPs for the previous set too. And finally, just to absolutely make sure I didn't miss them, I worked out the approximate dates, and set reminders on my calendar, so I knew I had to make an appointment around that time.
