When do I get MATB1 maternity form ?

Hi everyone, do you know when you get the MATB1 form? My work are chasing it, I've had a look through my pregnancy notes and can't figure it out. And google says 20 and 25 weeks.

I have my 20 week scan next Thursday and my 25 week midwife appointment on the 9th November - any ideas which appointment I'll get it?

I got mine at my 24/25 week midwife appointment. The form said it can't be given until at least 20 weeks.

I got mine at my midwife app at 25 weeks, which I think is pretty normal. If your work is pushing for it, you could contact your midwife to ask for it now?

Thanks everyone, probably should have mentioned why they are chasing!

I'm going on maternity leave after Christmas, and the lady at my work thats sorting my leave is also going on leave for a few months for chemo treatment - so she's just trying to sort it prior to her leaving.

I'll give the midwife a call after my 20 week scan, thanks so much x

They can’t fill it in before the 20 weeks so work shouldn’t be chasing you yet

My midwife posted mine to me after my 20 week scan. She had called me to book me in for my 25 week appointment, so I asked her then if she could, and it wasn't a problem.

I was given mine at my 20 week scan. I can see why that particular woman is chasing it at your work but she’s just going to have to accept there’s nothing happening until at least next week 🤷🏻‍♀️ I’m sure if you explain to the midwife next week they’ll get it done for you.

No-one mentioned the MATB1 form at my appointments, but when I asked the reception at the hospital they got an available midwife to come down and certify the form. If they don't mention it at your scan specifically, I definitely recommend just asking someone about it.
