Maternity Certificate

Hi Ladies,

Hope you are well. I have recently received MATB1 form from my midwife which has part A in it (hoping format is consistent) which is not filled up by her. Its says it should be filled before confinement.

Do i need to provide the due date in it before sharing with my employer ?

Weirdly no where on the certificate my due date is captured ! Please advise

Hmm on both matb1 certs I've had the midwife has filled that part in
