Help - back of MatB1 form

Help!
I have lost my MatB1 form. I had scanned it and gave it to my employer but evidently scanned the same side twice. My work didn't notice and accepted it but my husband's work are insisting on having a copy of the back and say it had details like my national insurance number and address.... I really don't remember this though.

Can anyone confirm what is on the back of the form? And is there a way to get a replacement? The govt website is not very helpful.

He doesn't need proof you are pregnant so they have no right to ask for a copy of your matB1

Why do his employer need your NI number? My husband has literally just told his boss I’m pregnant, they don’t need any written confirmation. Are you sharing your maternity leave or something?

Some employers do require proof of the partner's pregnancy before they approve paternity leave - one example being the NHS Trust I work for.

They shouldn't be allowed to ask for it if it specifically says on the .gov website that they don't need to see proof of pregnancy

The father’s employer has no right to ask for the MATB1. The mother is supposed to give the original to her employer (though these days most use a photocopy or scan).

They cannot refuse paternity leave or pay on that basis.
